About the Role:

As a Home Health Caregiver, you will play a vital role in delivering high-quality home health care services to our patients. Your primary responsibilities will include:

• Providing personal care and assistance with daily living activities, such as bathing, dressing, and grooming

• Assisting with medication management and administering medications as prescribed

• Maintaining a safe and healthy environment for patients, including assisting with meal preparation and housekeeping tasks

• Developing and implementing individualized care plans to meet the unique needs of each patient

• Communicating effectively with patients, families, and healthcare providers to ensure coordination of care

• Participating in ongoing education and training to stay current with industry best practices and standards
