Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home Health experience preferred.
• Experience with OASIS and HCHB preferred.
• Ability to perform in-home physical therapy evaluations and manage patient care across multiple counties.
• Must be able to travel within coverage area counties: Clinton, Marion, Washington, Jefferson, Hamilton.
• Minimum education requirement: Bachelor's Degree.

Assignment Details

• Facility type: Home Health.
• Shift Name: Days.
• Schedule: 4x8 (days).
• Weekly hour guarantee: 32 hours per week.
• On call/weekend requirement: No.
• Mileage reimbursement between patient visits from first to last visit: Yes.
• Coverage area includes multiple counties; candidate must be able to travel within the service area.
• Required certification: Illinois PT license.
