Job Description:

Requirements:

• Current Physical Therapist license in the state of practice.
• Graduate of an accredited Physical Therapy program.
• Experience in skilled nursing facility or geriatric rehabilitation preferred.
• Strong clinical, communication, and organizational skills.

Roles & Responsibilities:

• Evaluate residents' mobility, balance, strength, gait, and functional limitations.
• Develop and implement individualized treatment plans to improve physical function and reduce pain.
• Provide direct therapy interventions including therapeutic exercises, gait training, and balance retraining.
• Monitor residents' progress and adjust care plans as necessary to meet therapy goals.
• Educate residents, families, and caregivers on home exercise programs and safety strategies.
• Supervise and guide Physical Therapist Assistants and therapy aides in accordance with licensure requirements.
