Job Description
Title: Physical Therapist - Physical Therapy

Requirements:
- Current Physical Therapist license in the state of practice.
- Graduate of an accredited Physical Therapy program.
- Experience in skilled nursing facility or geriatric rehabilitation preferred.
- Strong clinical, communication, and organizational skills.

Roles & Responsibilities:
- Evaluate residents' mobility, balance, strength, gait, and functional limitations.
- Develop and implement individualized treatment plans to improve physical function and reduce pain.
- Provide direct therapy interventions including therapeutic exercises, gait training, and balance retraining.
- Monitor residents' progress and adjust care plans as necessary to meet therapy goals.
- Educate residents, families, and caregivers on home exercise programs and safety strategies.
- Supervise and guide Physical Therapist Assistants and therapy aides in accordance with licensure requirements.
