Medical Social Worker (2920) – Citywide – H00140 Department: Public Health Job class: 2920-Medical Social Worker Salary range: $115,362.00 – $140,192.00 Role type: Permanent Civil Service Hours: Full‑time Exam type: Class Based Test Rule: Rule of the List List type: Entrance Responsibilities Under general supervision, the Medical Social Worker performs routine medical social work duties, including evaluating social, emotional, and physical needs of clients; interviewing clients and their families; providing supportive counseling or crisis intervention; conducting mandatory reporting; advocating on behalf of clients; providing discharge planning; coordinating services; delivering psychoeducation; and performing related duties as required. The essential job functions include: Evaluate social, emotional, and physical needs of clients by applying social work theories, assisting clients in coping with environmental and psychological issues, explaining options and treatments, and making appropriate referrals. Provide supportive counseling or crisis intervention to clients and/or their significant others, families, or friends, and/or refer services to therapists or mental health providers. Interview clients, families, and significant others to obtain intake information, review financial status, and screen eligibility for entitlement programs; collaborate with interdisciplinary health care teams to plan psychosocial and medical needs; devise, develop, and implement treatment plans; may make a diagnosis as part of a treatment plan. Conduct mandatory reporting activities, completing reports for CPS, APS, elder abuse, and domestic violence as required. Prepare records to document case activity, including statistical reports, chart notes, correspondence, and other agency forms. Advocate on behalf of clients with agencies, medical practitioners, and the community. Conduct home visits when necessary to assess living situation for accessibility and determine need for additional services. Provide discharge planning and service coordination, analyze client needs, coordinate with treatment teams, refer clients to outside services and community agencies, research resources, make referrals, and facilitate follow‑up care. Qualifications Minimum Qualifications Education: Possession of a Master’s Degree in Social Work (MSW) from a fully accredited school of social work by the Council on Social Work Education (CSWE). Experience: Six (6) months (1,000 hours) of social work experience in a health care setting. Experience may be gained from supervised field placement during the MSW program or from post‑graduate experience. Applicants who have qualifying experience but do not yet have their MSW degree may qualify if they upload documentation showing current enrollment in the final academic term of a qualifying MSW program, and the program will award the degree by June16,2026. However, hiring will not occur until proof of MSW completion is provided. Desirable Qualifications Comfort responding as float in urgent social work requests in varied hospital outpatient units. Experience in urgent care and/or dialysis setting. Willingness to provide case management services (on‑campus only). Excellent communication skills. Able to manage multiple tasks or assignments. Bilingual in Spanish or Cantonese.
