Care Management Technician

The Care Management Technician is focused on supporting the care management team in achieving the department goals and objectives of patient transition/throughout. Responsibilities include educating patients/families on the roles of the care management team, bridging communications with the other members of the health care team, and ensuring timely transitions to the next level of care. The technician assists with care management requests, arranges nursing home returns, contacts SNF's, ALF's, and rehabilitation centers to refer to and ascertain bed availability, identifies availability of community resources, enrolls patients in patient assistance programs, refers to community agencies, arranges patient transportation, obtains authorization numbers from payors, arranges for resources to facilitate durable medical equipment, and issues Important Message from Medicare (IMM) letters, Observation notifications, Advance Beneficiary Notices (ABNs), and Hospital-Issued Notices of Noncoverage (HINNs). The technician also assists in overall office functions, maintains required files, statistics, and reports, maintains databases and spreadsheets, schedules meetings, coordinates arrangements for meetings, suggests, develops, and implements new processes to improve department efficiencies, provides timekeeping support, participates in quality and safety initiatives, prioritizes workload, and maintains regular, punctual attendance consistent with Orlando Health policies.

Qualifications include a high school graduate or equivalent, two years' experience in working with customers and/or patients, including one year in a health-related field, and working knowledge of Microsoft Office programs (Word, Excel, Outlook, PowerPoint).
