Ultimate Therapy is hiring a Physical Therapist (PT) to join our rehabilitation team in a skilled nursing setting. We are seeking a clinician who is passionate about helping residents restore mobility, improve physical function, and return to the highest level of independence possible.

What You'll Do:

Complete physical therapy evaluations and develop individualized care plans
Provide treatments focused on strength, balance, gait training, endurance, and pain management
Work closely with PTA staff and interdisciplinary teams to coordinate resident care
Educate residents and families on mobility strategies, safety, and home exercise programs
Monitor patient progress and update treatment plans as needed
Maintain accurate and timely clinical documentation

Qualifications:

Active Physical Therapist license required
Strong communication and clinical reasoning skills
