Active Ohio Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Experience in home health settings preferred. Ability to meet productivity expectations and weekly hour guarantee as outlined by the client (weekly guarantee: 36 hours; productivity expectations: 30 points). Valid driver’s license and reliable transportation for patient visits. Mileage reimbursed between visits from first patient to last patient. Professional appearance consistent with client dress code. Preferred scrub colors: gray or black. Minimum education requirement: Doctoral preferred by client. This is client-specific and may be required. Assignment Details Facility type: Home Health. Shift: Days. Shift hours: 08:00–16:00 (client shift schedule lists 8:00 am–4:30 pm; core shift provided as Days 0800-1600). Work week: 5x8 schedule. Weekly hour guarantee: 36 hours. On-call/weekend requirement: No. Coverage area: Office service area; travel between patient homes required. Required certification: Physical Therapist license.
