Requirements *SCP APPROVED FOR IMLC AS OF 07/08/2026* OhioHealth Mansfield-EM GFID 4525 335 Glessner Ave Mansfield, OH Rate Cap: $400/hour 40k annual volume ADE 109 Admit/Obs: 30% Transfer: 3.1% EMS Arrivals: 33% Coverage: 37.28 MD and 27.57 NPPA Shift Times MD: 6:30a-4p, 10a-9p, 3p-12a, 9p-7a Monday 6:30a-4p, 10a-9p, 4p-12a, 9p-7a Tuesday 6:30a-4p, 11a-9p, 4p-12a, 9p-7a Wednesday-Friday 7a-4p, 11a-9p, 4p-12a, 9p-7a Saturday & Sunday NPPA: 8a-6p, 11a-10p, 6p-4a Monday Thursday 8a-6p, 1p-10p, 6p-4a Friday 8a-7p, 6p 4a Saturday & Sunday EPIC EMR Orientation: EMR Training and 1 Shadow shift 45 ED beds and 5 triage rooms Level 3 trauma center Backup/Specialty Services Surgery, Neuro, Cardiology, OBGYN, Ortho Physician Requirements: BC/EM/FM/IM with 2 years EM experience Active OH license and DEA required No Temps Allowed 90 day privileging timeframe once app is complete and submitted to CVO Completed app, COI and all required credentials, verification of all education/training/licensure/boards, claims history of past 5 years, all current/prior work history and affiliations verified, three peer references received for file to move forward Items Needed: State license and DEA, Copies of Degree, Residency certificate, board certificate, photo, driver's license, TB, Flu ABEM providers do not need to maintain any certs, must show documentation of having ATLS once in the past ABFM/ABIM must maintain ATLS
