To learn more about our values, please read our Statement of Faith. \nJob Title: CSEY Care Coordinator\nJob Status: Full-time, exempt, grant-funded\nJob Location: Travis County, Hybrid\nJob Summary: Unbound Now, with the endorsement of county community partners and the financial support and direction of the Office of the Governor’s Child Sex Trafficking Team, is committed to implementing the Texas Model for Care Coordination for Commercially Sexually Exploited Youth (CSEY). Care Coordination facilitated by Unbound Now is consensus-driven and collaborative, with a focus on identifying and supporting CSEY and connecting youth and families to tailored, accessible, trauma-informed, and holistic resources through a coordinated network of community partners. The goal is for every identified youth survivor of sex trafficking to have access to non-punitive, responsive, high-quality, community-based services that address their unique short- and long-term needs. Care Coordination includes awareness, education, collaboration, continuous learning, creativity, and capacity-building to strengthen the community’s ability to identify and support CSEY. Care Coordination teams build trust, transparency, and collaborative solutions among partners to reduce duplication of efforts and ensure that local and statewide systems serve as bridges rather than barriers to services for youth and their families.\nThe primary functions of the CSEY Care Coordinator are to implement Unbound Now’s Care Coordination program as described above; facilitate county-level consensus-building and protocol development; and ensure compliance with Unbound Now policies and procedures and the expectations of the Texas Office of the Governor’s Child Sex Trafficking Team. Responsibilities include participating in shared 24/7 crisis response coverage with other Care Coordinators; completing CSE-ITs as needed; securing and maintaining appropriate releases of information and consent for Care Coordination services; facilitating Rapid Response Meetings, Service Status Meetings, and Family Empowerment Meetings; developing and maintaining strong relationships with community partners; ensuring timely and accurate documentation; supporting and facilitating awareness, education, and training opportunities; conducting case analyses and data evaluation sessions; and coordinating and scheduling Advisory Council meetings.\nCompensation: Annual salary\nBenefits: Unbound Now is proud to offer a premier benefits package that prioritizes the health and financial security of our team members and their families:\n\n100% Employer-Paid Health Insurance: We cover 100% of medical premiums for both the employee and their family. \nEmployer-Paid Life Insurance: Fully covered for the employee at no cost.\nRetirement Savings: 401(k) plan with a 3% salary match (eligible after 90 days; 1-year vesting).\nDental & Vision: Comprehensive plans available at the employee’s expense.\n\nAvailability: Generally, Monday through Friday, 8:30-5:30. Anticipated 45-50-hour workweek. \nTravel: This position requires travel throughout the service region and may require limited travel for training and team/professional development opportunities\nWorking Conditions: Work performed primarily remotely, with some expectation and flexibility of work in normal office environments as required. The job requires the ability to respond remotely during the night, as well as attention to detail and the ability to document in an electronic case management system.\nJob Responsibilities:\n\nBuild consensus among county community partners to establish and maintain Care Coordination protocols consistent with the Texas Model for Care Coordination.\nImplement Unbound Now’s Care Coordination program in accordance with the expectations of the Texas Office of the Governor’s Child Sex Trafficking Team (CSTT), applicable grant requirements, Unbound Now policies and procedures, and subsequent CSTT guidance.\nParticipate in shared 24/7 remote crisis response coverage with other Care Coordinators in accordance with established Care Coordination Team (CCT) protocols.\nComplete CSE-ITs as needed and support appropriate responses based on screening results.\nSecure and maintain appropriate releases of information and consent for Care Coordination services and safeguard confidential information in accordance with applicable requirements.\nEncourage engagement with CSEY Advocacy services and other appropriate community-based supports.\nFacilitate Rapid Response Meetings (RRMs), Service Status Meetings (SSMs), Family Empowerment Meetings, and other Care Coordination Team meetings as appropriate.\nSchedule, coordinate, and facilitate regular Advisory Council meetings.\nSupport the Care Coordination team in promoting and hosting community awareness, education, and training opportunities.\nFacilitate case analysis and data evaluation sessions with Care Coordination teams and Advisory Councils as appropriate.\n\nDevelop and maintain strong working relationships with essential community partners, including but not limited to children’s advocacy centers, CASA, CSEY Advocacy agencies, DFPS, Community-Based Care providers, medical providers, juvenile justice/probation, law enforcement, district attorney’s offices, and other community-based service providers.\nPromote partner commitment, consistency, accountability, collaboration, and information sharing, and seek feedback from community partners to strengthen the coordinated response.\nMaintain regular communication with the Care Coordination Program Director and/or Assistant Program Director regarding program implementation, challenges, partner concerns, significant case issues, and other matters requiring leadership support.\nParticipate in individual and group supervision, team meetings, and regular gatherings of Unbound Now Care Coordination staff to review program progress, discuss upcoming objectives, strengthen practice, and promote consistency in service delivery.\nRepresent Unbound Now and communicate effectively about Care Coordination services with community partners and other stakeholders as needed.\nDocument referrals, intakes, meetings, service plans, outgoing referrals, communications, action plans, and other required case activity promptly and accurately in Unbound Now’s electronic case management system, generally on the same day.\n\nCrisis Response and Rapid Response Responsibilities:\n\nRespond to crisis referrals in accordance with established CCT protocols, including obtaining appropriate consent and contacting the designated CSEY Advocacy agency.\nCoordinate with law enforcement, DFPS, medical providers, and other CCT partners following the identification or recovery of a youth, as appropriate.\nNotify appropriate CCT members of an identification, recovery, or qualifying CSE-IT result in accordance with established protocols.\nInitiate the appropriate Care Coordination case record and begin collecting relevant information from DFPS, juvenile justice/probation, law enforcement, the children’s advocacy center, CSEY Advocacy, and other partners as needed.\nCoordinate Rapid Response Meetings by notifying participants, scheduling and facilitating meetings, documenting decisions, and developing and distributing action plans.\nComplete follow-up activities resulting from Rapid Response Meetings, including coordinating with identified service providers and following up with responsible referral sources as appropriate.\nMaintain communication with CSEY Advocacy and other partners directly supporting the youth to obtain relevant updates that inform Care Coordination Team planning and decision-making.\nSchedule and facilitate Service Status Meetings for assigned cases and coordinate information sharing among CCT/MDT partners.\n\nMaintain responsibility for the coordination, documentation, follow-up, and ongoing management of assigned Care Coordination cases.\n\nCommunity and Advisory Council Responsibilities:\n\nDevelop and maintain strong relationships with Advisory Council members and other community partners.\nSupport, promote, and host education, training, and awareness opportunities.\nCoordinate and facilitate Advisory Council meetings in partnership with Care Coordination leadership and team members.\nConduct case analysis and data evaluation sessions to identify trends, gaps, strengths, and opportunities to improve the community’s coordinated response to CSEY.\n\nOrganizational and Administrative Responsibilities:\n\nRespond appropriately to allegations of abuse, including youth-to-youth sexual activity, by taking all allegations seriously, following applicable mandatory reporting requirements, and immediately notifying the Care Coordination Program Director as required.\nComplete all required Unbound Now training within established time frames.\nSubmit expense documentation accurately and within required time frames in accordance with Unbound Now expense policies and applicable Ramp Monthly Closeout procedures.\nSubmit travel reimbursement documentation in accordance with Unbound Now travel policies and established timelines.\nSubmit timesheet hours and grant allocations accurately and within required time frames in accordance with organizational and grant requirements, as applicable.\nPerform other duties related to Care Coordination program implementation and service delivery as assigned.\n\n*This position description is not designed to cover or The scope of the position may change as necessitated by organizational demands.\nDesired Outcomes:\n\nYouth and their families are consistently served with professionalism, compassion, and a trauma-informed approach.\nCare Coordination services are implemented in alignment with the expectations of the Texas Office of the Governor’s Child Sex Trafficking Team (CSTT), applicable grant requirements, Unbound Now policies and procedures, and established county protocols.\nStrong, collaborative, and effective working relationships are developed and maintained with county community partners.\n\nCare Coordination meetings, case activities, referrals, service plans, and other required documentation are entered accurately and promptly into the electronic case management system to support service delivery, program evaluation, and reporting requirements.\nCommunity partners understand the purpose, role, and services of Unbound Now’s Care Coordination program and how to access Care Coordination when a youth is identified or recovered.\nAdvisory Councils and Care Coordination Teams demonstrate effective collaboration, communication, accountability, and shared commitment to strengthening the community’s coordinated response to CSEY.\nCare Coordination is represented professionally through presentations, trainings, meetings, written materials, and other community engagement opportunities.\n\nWorking Relationships:\nSupervisor: Care Coordination Program Director \nWorks with: Assistant Care Coordination Program Director, other CSEY Care Coordinators, Unbound Now staff, Advisory Council members, Care Coordination Team members, and county community partners.\nExperience and Education:\nRequired Qualifications:\n\nBachelor’s degree in Social Work, Criminal Justice, Psychology, Sociology, Human Services, or a related field; or an equivalent combination of education and professional experience.\nDeep knowledge of trauma-informed, youth-centered, and strengths-based practices, with direct experience supporting youth and families impacted by trauma, abuse, exploitation, or complex circumstances.\nDemonstrated experience collaborating with multidisciplinary teams (MDTs), community partners, and youth-serving systems to build relationships, engage diverse stakeholders, navigate differing perspectives, achieve consensus, and facilitate collaborative decision-making.\nDemonstrated ability to facilitate meetings, group discussions, community presentations, or professional trainings.\nExcellent verbal and written communication skills in sensitive environments, with the ability to respond effectively and professionally during crises.\n\nStrong organizational skills to manage competing priorities, maintain timely and accurate documentation, work independently, and exercise sound professional judgment.\n\nPreferred Qualifications:\n\nExperience working directly with commercially sexually exploited youth (CSEY), survivors of human trafficking, or youth at high risk of exploitation, backed by relevant specialized training.\nExperience within child welfare, children’s advocacy centers, juvenile justice, law enforcement, victim services, or coordinated community responses/MDTs.\nExperience facilitating or developing interagency protocols, procedures, or collaborative agreements.\nExperience using electronic or cloud-based case management systems.\n\nJob Requirements:\n\nActive mature Christian faith, as evidenced by participation in a local church, and commitment to abiding by all Unbound Now policies.\nPass employment eligibility verification, provide three references (supervisor, professional, personal), and clear all required criminal background, DFPS history, and abuse/neglect registry checks.\nStrong interpersonal and verbal communication skills, including cultural competency, effective consensus-building, and public presentation skills (in-person and online).\nExcellent organizational abilities, creative problem-solving skills, strict commitment to confidentiality, and the flexibility to operate in both structured and unstructured settings.\nTimely submission of daily time-sheets/grant allocations, daily travel reimbursements, and monthly expense documentation following company guidelines.\nWillingness to travel regionally using a reliable personal vehicle with a valid driver's license and insurance, and prompt completion of all required staff training.\n\nPhysical and Driving Requirements\n\nDemands include sitting for long periods at a computer and frequent video calls.
