Experienced Home Health Physical Therapist (PT)

One Home Health serves patients throughout Porter County and surrounding communities in Northwest Indiana . Multiple service territories are available, and assignments are made based on patient needs, clinician preference, and geographic location whenever possible.

Compensation

Job Type: Part-Time, PRN/Per Diem
Pay: $60.00 - $80.00 per hour
Location: Porter County and Surrounding Northwest Indiana Communities

Compensation is based on experience, qualifications, and clinical expertise.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Provide skilled physical therapy interventions according to physician orders and established care plans.
• Assess patients' home environments and recommend safety modifications and assistive devices as appropriate.
• Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
• Monitor patient progress and adjust treatment plans based on clinical findings.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in discharge planning and patient outcome evaluations.

QualificationsRequired

• Active Illinois Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Excellent communication, organization, and time-management skills.
• Ability to work independently in a home health setting.

Preferred

• Home health experience preferred.
• Experience with Medicare home health regulations and documentation requirements.
• Knowledge of OASIS documentation.
• Experience with Axxess software.
• Strong orthopedic, neurological, geriatric, or rehabilitation background.
• Bilingual candidates are encouraged to apply.
