Home Health Physical Therapist (PT) Provide personalized, home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury in the Sacramento area. Collaborate with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement individualized care plans. Assess patient progress through observations and documentation, adjusting treatment strategies as needed. Assist patients with exercises and hands-on therapy to manage pain and improve mobility. Educate patients and their families on care plans, exercises, and safety to promote self-care and independence. Maintain compliance with state and federal home health regulations, demonstrating strong clinical judgment and interpersonal skills. Qualifications Include Graduation from an accredited Physical Therapist Program and active PT license in CA At least 1 year of experience, preferably in home healthcare CPR certification Knowledge of home health regulations Compassion, dexterity, and strong clinical and interpersonal skills #J-18808-Ljbffr
