Job Description Job Overview Join Elevate Care Providers LLC as a Private Duty Nurse (RN/LPN) providing one-on-one skilled nursing care in the comfort of a patient’s home. This role offers consistent hours, a low-stress environment, and the opportunity to build meaningful relationships while delivering high-quality, patient-centered care. As a Private Duty Nurse, you will support patients with medically complex needs while promoting safety, comfort, and independence in a home setting. You’ll work closely with the patient’s care team and family to ensure continuity of care and excellent outcomes. Responsibilities Provide skilled one-on-one nursing care in the home setting Administer medications as ordered Perform G-button / tube feeding administration Provide nebulizer treatments and suctioning Monitor patient condition and report changes promptly Assist with bathing, dressing, hygiene, and other ADLs as needed Follow the physician’s plan of care and nursing orders Maintain accurate and timely nursing documentation Communicate effectively with families, care coordinators, and supervisors Ensure infection control and patient safety at all times Patient Care Needs Ventilator-dependent care Tracheostomy care and suctioning Respiratory monitoring and support Medication administration Urostomy care Bowel program Passive Range of Motion (PROM) Assistance with ADLs (bathing, feeding, positioning) Qualifications Active Missouri RN or LPN license (required) Experience in private duty nursing, home care, or skilled nursing preferred Comfortable with G-tube care, suctioning, and medication administration Strong assessment and communication skills Compassionate, dependable, and patient-focused Ability to pass background screening and meet state requirements Schedule & Hours Flexible scheduling available Days, nights, and weekends may be available Why Work With Elevate Care Providers LLC One-on-one patient care (no facility ratios) Consistent hours on a stable case Supportive, responsive management Opportunity for long-term placement Benefits Referral program License/Certification RN License and LPN (Preferred) Job Types: Full-time, Part-time, PRN, Per diem Work Location: In person #J-18808-Ljbffr 6AM City, LLC
