Work Environment As a Physical Therapist at Oasis Physical Therapy, you will work onsite within patients' homes, which allows for an individualized approach to treatment. Key Responsibilities Conduct thorough evaluations of patients' physical capabilities, developing personalized treatment programs based on their unique needs and goal orientations. Design treatment interventions that may include therapeutic exercises, manual therapy techniques, and modalities such as ultrasound and electrical stimulation. Regularly monitor patient progress, adapting treatment plans and approaches as necessary to maximize outcomes, and accurately document all treatment procedures and adjustments in compliance with regulatory standards. Collaborate with a multidisciplinary team, involving physicians and other healthcare professionals to integrate physical therapy with overall healthcare strategies. Provide education to patients and their families regarding best practices in home exercise programs and self-management techniques for ongoing physical health. Ensure adherence to local, state, and federal regulations, maintaining high professional standards and practices. Patient Population / Client Focus Your primary focus will be on diverse patient populations, including elderly individuals recovering from surgery, those managing chronic conditions, and individuals rehabilitating from injuries. Working closely with the geriatric population, you will be instrumental in helping patients regain independence and improve their functionality in everyday tasks, which can significantly influence their quality of life. Qualifications Current licensure as a Physical Therapist in New York State is mandatory. A Bachelor's Degree in Physical Therapy is required; however, candidates holding a Master's or Doctorate will be given priority consideration. Proficiency in assessment, planning, and implementation of comprehensive treatment strategies. Strong communication skills, with the ability to effectively engage with patients, families, and interdisciplinary team members. Organizational skills to manage patient schedules and documentation efficiently. A commitment to engaging in continual professional development and ongoing education in therapy practices. Preferred Experience At least 2 years of practical experience in a physical therapy setting is required, with preferred experience within home health or skilled nursing environments dealing with geriatric patients. Experience in community-based rehabilitation programs is highly regarded.
