Associate Patient Care Coordinator

Title: Associate Patient Care Coordinator
Duration: 3 months contract + Possibility of Extension
Location: FT WORTH, TX 76104
Shift: : Day Shift

Roles & Responsibilities:
Answer and manage a high volume of inbound patient calls in a professional and timely manner
Obtain and document detailed and accurate information in the electronic medical record (EMR)
Route messages and clinical concerns to the appropriate provider, nurse, or department based on established protocols
Assist patients with general inquiries and provide exceptional customer service
Schedule, reschedule, and confirm patient appointments
Coordinate appointment requests and provider availability
Conduct appointment reminder and confirmation calls to reduce missed appointments
Ensure patient demographic and insurance information is current and accurate

Minimum Qualifications:
High School Diploma or GED equivalent
Previous customer service, medical office, healthcare administration, or call center experience
Strong telephone communication and interpersonal skills
Ability to accurately document patient information and conversations
Proficiency with computer systems and electronic medical records
Strong organizational and multitasking abilities
