Street Medicine CHW: Community Outreach & Advocacy

The Department of Family Medicine at the Keck School of Medicine of USC seeks a Community Health Worker for the Street Medicine Program in Los Angeles. This full-time, on-site role supports patient navigation, social referrals, and coordinated care with the medical team. Some after-hours work and driving responsibilities are involved. Candidates should have a high school diploma, 1 year of relevant experience, and a California driver’s license.
