Proviso Leyden Council for Community Action (PLCCA) seeks a Family Service Worker.

Qualifications

• Bachelor's degree in social services, human services, adult education, family services, early childhood education, or another related field is required.
• Minimum of three years experience working with adults; and in the early learning community.
• Minimum of three years experience collecting, organizing, recording, and processing data information.
• Valid driver's license and reliable transportation, with appropriate insurance coverage; Cleared state and federal background check and drug test.
• Physical examination upon employment and within every twenty-four (24) months thereafter; Tuberculosis test upon employment and within every twenty-four (24) months thereafter.

Principle of Duties and Responsibilities

• Eligibility, Recruitment, Selection, Enrollment, Attendance (ERSEA)
• Implement, document, and report on the recruitment, selection, and enrollment of potentially eligible EHS-CCP infants and toddlers and their families;
• Determine eligibility, processes enrollment, transfer, and termination of EHS-CCP children, including obtaining all records for enrollment, sharing information with parents/guardians, recording information about enrollees.
• Receive and process referrals, including eligibility, selection, and enrollment; monitor children's participation and report on these activities; Report on and follow up with any families experiencing attendance problems.

Salary: $40,000 Annually
