FreedomCare in Missouri is seeking a Health Coach Specialist II to provide orientations, assessments, and caregiver training. This field-based role requires travel within the Kansas City area, with 32 hours of in-field visits and 8 hours of administrative work from home each week. The ideal candidate has 2+ years of elder care experience, strong communication, and is comfortable coordinating with care managers and families.
