Description

• Assess patients in both inpatient and outpatient contexts and craft personalized treatment plans
• Deliver physical therapy interventions targeting mobility, strength, stability, and overall well-being
• Work closely with a team of nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Educate patients and their families on therapeutic objectives, discharge processes, and exercises for home
• Maintain accurate records of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
• Engage in interdisciplinary care rounds and strategy meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory
