The Hospice Liaison/Hospice Sales Rep generates referrals and drives census growth by building relationships with physicians, hospitals, skilled nursing facilities, and community providers. This role educates partners about hospice services, facilitates timely referrals and smooth care transitions, and collaborates with the interdisciplinary team while maintaining a compassionate, patient-first approach. Primary Responsibilities:Educates healthcare professionals, patients, and families about home care servicesDevelops and maintains relationships with key referral sources, such as hospitals, nursing homes, and physician offices to generate leadsCoordinates patient admissions with the team, including obtaining all required documentation to complete the referral-to-admission processConducts on-site visits to assess patient needs and identify barriers to careKeeps accurate records of patient interactions and referralsAchieves admissions, product, and payor-mix goalsMeets short and long-term target account goalsDocuments the minimum expectation of sales calls daily in CRM (including pre- and post-call notes)Maintains a complete, up-to-date record of targeted referral sources in territory in CRMAttends community events to promote AmedisysResponds to customer complaints in a timely mannerPerforms other duties as assignedTravel 50% or more within the assigned territory You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:1+ year of experience in a healthcare-related business development/outreach roleReliable transportation, current driver's license, and liability insurance Preferred Qualifications: Bachelor's degree Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $65,600 to $98,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
