- Provide ongoing case management services and individualized support to residents.
- Connect residents with community resources, benefits, healthcare services, housing assistance, employment resources, and other supportive programs.
- Assist residents in developing and achieving personal goals through advocacy, coaching, and resource coordination.
- Maintain accurate case notes, documentation, and resident records.
- Conduct resident assessments and monitor progress toward established goals.
- Collaborate with internal staff and external service providers to ensure comprehensive support services.
- Occasionally transport residents to appointments, meetings, and community resources as needed.
- Bilingual Spanish/English required
- Experience providing nonprofit, education/childcare, advocacy, social services support preferred.
- Excellent communication, organization, and interpersonal skills.
- Ability to work independently while collaborating effectively with a team.
- Flexibility and adaptability in a fast-paced environment.
- Proficiency with Microsoft Outlook, Word, and basic Microsoft Office applications.
- Valid driver's license and ability to transport residents as needed preferred.
