Registered Nurse Home Health Case ManagerRegistered Nurse Home Health Case Manager provides skilled nursing care and coordinates individualized care plans for patients in their homes. This role supports interdisciplinary teams and ensures compliance with home health and Medicare regulations. The position is based in Boise, Idaho.Key Responsibilities:Provide skilled nursing care in patients' homes, promoting safety, independence, and quality of lifeServe as primary RN Case Manager, coordinating overall care plans for assigned caseload and interdisciplinary teamPerform comprehensive assessments, evaluating physical, psychological, and social needsDevelop, implement, and update individualized care plans based on needs and physician ordersComplete accurate, timely documentation per agency, state, and federal standards, including OASIS assessmentsMonitor patient progress, identify changes requiring intervention or physician notificationEducate patients and families on medications, disease processes, treatment plans, symptom management, and home careReview medications, identifying potential concerns or discrepanciesCommunicate effectively with patients, families, physicians, and other professionalsParticipate in case conferences and coordination activities as neededPromote continuity of care throughout the home health episodeMaintain compliance with home health, Medicare, state, and organizational standards while managing time effectively between patient homes and documentationQualifications:Current and unrestricted Registered Nurse RN license in Idaho or eligibility for Idaho licensureMinimum 1 year of Home Health RN experienceCurrent BLS/CPR certification for healthcare providersAssociate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from accredited programValid driver's license and reliable transportationAbility to maintain appropriate automobile insuranceStrong clinical assessment, communication, organization, and critical-thinking skillsHome health, Medicare, OASIS, and care coordination experience strongly valuedPreferred Skills:Previous experience as a Home Health RN Case ManagerExperience completing OASIS assessments and documentationFamiliarity with Medicare home health regulations and requirementsExperience coordinating interdisciplinary patient careFamiliarity with home health electronic medical records and documentation systemsAbility to work independently while remaining connected with the clinical teamCompassionate, patient-focused approach to nursing care
