Job Title: Human Services Case Manager Agency: Department of Human Services Salary Range: $5,193 - $6,904 Position Type: Employee Job Description Step into a pivotal position that combines compassion with expertise, facilitating Medicaid eligibility and coordinating essential services to enhance the quality of life for individuals requiring long-term care support. Summary of Duties In this position you will determine initial and ongoing Medicaid eligibility for long-term services and supports (LTSS), develop and implement service plans, coordinate provision of services for community-based care and nursing homes, and regularly review and update Medicaid eligibility and monitor service plans. Minimum Qualifications A bachelor's degree in a Behavioral Science, Social Science, or a closely related field. A bachelor's degree in any field AND ONE YEAR of human services related experience (e.g., assistance to individuals and groups with economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing issues). An associate degree in a Behavioral Science, Social Science or a closely related field AND TWO YEARS of human services related experience (e.g., similar work as above). Three years of human services related experience (e.g., similar work as above). Essential Attributes Excellent communication skills to discuss/assess client needs. Customer service skills to provide excellent people service. Ability to obtain information through interviews. Skill in preparing and maintaining written documentation of actions taken and needed. Working Conditions The office is open from 8 am‑5 pm, Monday through Friday. The role requires physical exertion such as climbing stairs, walking on uneven terrain, transporting a laptop, and traveling to individuals’ homes, hospitals, nursing homes, provider settings, and local offices in Salem. Environmental hazards include unsanitary conditions. May encounter hostile or angry individuals who could pose verbal, physical, or weapon threats. May also encounter animals, some of which can be dangerous. Must drive a variety of vehicles, often in remote terrain and possibly inclement weather; exposed to contagious diseases such as staph infections, lice, MRSA, COVID‑19, etc. Must possess a valid driver’s license and maintain an acceptable driving record. Background Checks and Requirements Selected finalists will undergo a criminal history and background check; adverse history may lead to disqualification. ODHS does not offer visa sponsorship. Within three days of hire, the candidate must complete the US Department of Homeland Security’s I‑9 form and authorise use of E‑Verify.
