Required Skills & Experience Bachelor's degree in Social Work, Psychology, Human Services, or a related field 3+ years of experience in homeless services , housing programs, case management, or related social services Experience managing client caseloads and developing individualized service plans Strong understanding of Trauma-Informed Care, Housing First, Motivational Interviewing, and Harm Reduction principles Experience providing crisis intervention and conflict resolution Previous lead, supervisory, or program coordination experience Nice to Have Skills & Experience Master's degree in social work, Psychology, Counseling, or a related field Experience within interim housing, shelter programs, or homelessness prevention services Background in program development, quality assurance, or contract compliance Bilingual in English and Spanish Job Description The Lead Case Manager serves as the primary point of contact for client services and daily program operations within an interim housing environment. This individual manages a caseload of clients, conducts assessments, develops individualized service plans, and provides ongoing support through crisis intervention, referrals, and resource coordination. They collaborate with internal staff, volunteers, clinical professionals, employment specialists, and external partners to ensure each client receives coordinated, high-quality services. On a daily basis, the Lead Case Manager facilitates communication among stakeholders, leads program coordination meetings, monitors client progress, and ensures accurate documentation and compliance with program requirements. The role also supports quality assurance efforts, evaluates service delivery, identifies areas for program improvement, and helps maintain policies and procedures. Additionally, the Lead Case Manager builds and maintains community partnerships, connects clients with critical resources, and supports facility operations by addressing resident concerns, ensuring adherence to program guidelines, and assisting in crisis response situations as needed. #J-18808-Ljbffr Insight Global
