Family Nurse Practitioner – Salem, Oregon This position qualifies for the Oregon Loan Repayment Program. Salary: $135,000–$140,000 based on Experience and FTE. $12,000 Sign On Bonus, paid first pay period following 90 days of employment with OIH. Oregon Integrated Health Oregon Integrated Health is an integrated primary care medical home that is hiring a full-time nurse practitioner for our clinic in Salem, Oregon. We will be moving into a larger space Jan 1, 2025. Currently a Tier 4 PCCMH, Vaccine for Children Program and candidate for Rural Health Clinic designation. We have an amazing team and are looking forward to growth in Salem. Our Salem clinic serves a Medicaid and Medicare demographic needing community resources. The medical home has a team of Family Nurse Practitioners, Naturopathic Physicians, Licensed Professional Counselors, Behavioral Health Clinicians, Chiropractor, Acupuncture, Medical Assistants, Front Desk and community health workers with our Senior Director of Clinical Operations. Patients have access to Interpath Laboratories directly on site. We utilize telemedicine visits for patients at home. When patients lack data or smart phones, they come to our Portland facility to connect with providers across our 4 locations. Additional locations in Florence, Salem and Portland have Family Nurse Practitioners, Naturopathic Physicians, Psychiatric Mental Health Nurse Practitioners, Counselors, Chiropractors, Acupuncture, Physical Therapy, Massage, and oral health with an Expanded Practice Dental Hygienist. Job Details Sign On Bonus: $12,000, paid first pay period after 90 days of employment with OIH. Salary: $135,000–$140,000 based on Experience and FTE. Flexible hours and scheduling. Protected Sick Time: accrues 1 hour per 30 hours worked, max use of 40 per year, available immediately as time accrues after first day of employment. Job Duties and Responsibilities Provide primary care in an integrated setting with other specialties at the clinic. Assist with Medical Assistants who room patients, collect vitals, submit referrals and perform electronic orders as submitted by provider. Work with Patient Health Navigators that follow up on ED visits, referrals, complex care patients and quality metric data. New patient visits are 45 minutes, follow-up 30 minutes; urgent appointments 15 minutes; same day appointments available for each provider. Conduct standard medical and physical examinations and perform standard diagnostic tests in‑office and via telemedicine conferencing. Order, review and provide patient instructions regarding diet, activities and modifications as appropriate. Refer patients to other health professionals in treatment of diseases requiring further diagnosis and evaluation. Provide appropriate medical record documentation in an EHR on all patients in accordance with clinic policy. Qualifications, Skills and Abilities Previous experience in urgent or family care; emergency preferred. Comfortable and skilled with electronic platforms such as EHR. Accredited medical school graduate. State of Oregon license. Full prescription authority. Drug Enforcement Agency certificate.
