Schedule: 7 on/7 off rotating schedule; Saturday & Sunday 8:00 AM – 8:00 PM or 8:00 PM – 8:00 AM; Monday – Friday 5:00 PM – 8:00 AM; Standard 40 hours per week guaranteed

Responsibilities:

• Assess patient and family physical, psychosocial, environmental, safety, and developmental needs
• Develop and implement individualized pain management plans and comfort care interventions
• Perform hospice admissions and coordinate all patient and family services
• Document nursing assessments, goals, care interventions, and patient outcomes accurately and timely
• Educate patients, caregivers, and families on medications, interventions, and symptom management
• Maintain dignity of dying patients while supporting unique spiritual and cultural beliefs
• Collaborate with interdisciplinary teams to ensure comprehensive, quality end-of-life care

Requirements:

• Active RN license in Texas
• Extensive hospice nursing experience required
• Demonstrated experience in hospice on-call and admissions processes
• Proficiency with HCHB (Home Care Home Base) electronic medical record system
• Strong assessment, clinical decision-making, and communication skills
• Ability to work independently and respond to urgent patient needs
