Physical Therapist

Responsibilities Provides direct patient care, evaluates functional needs and outcomes, and establishes plans of care. Conducts therapy practice within defined standards of care, evaluates effectiveness of treatment, and modifies plans as necessary. Assesses educational and cultural needs of patients, families, and caregivers and communicates accordingly to maximize benefit of care. Documents evaluations, treatment goals, and plans in accordance with hospital requirements and established guidelines; maintains prescriptions and signatures. Prepares discharge plans and coordinates with interdisciplinary team on patient needs, treatment modifications, and progress. Attends and participates in weekly team conferences. Coordinates, delegates, and supervises responsibilities assigned to support staff. Provides an environment conducive to safety for patients, visitors, and staff, assessing risks and implementing precautions. Complies with appropriate and approved safety and infection‑prevention standards. Maintains regular attendance in keeping with assigned work schedule. Performs other duties as assigned to support overall effectiveness of the organization. Qualifications Minimum Education & Experience : Two years of direct patient physical therapy care and treatment experience; experience in a rehabilitation hospital setting preferred. Current state licensure as a Physical Therapist required. Basic Life Support/CPR certification required. Demonstrates appropriate application of age‑specific standards, policies, procedures, and guidelines for pediatric, adolescent, adult, and geriatric patients. Knowledge of accreditation standards to ensure adherence to state and accrediting agencies (TJC and CMS). Knowledge of clinical operations and procedures and utilization of proper policies and guidelines in patient care. Understanding of treatment costs and financial support as they relate to quality and efficiency. Physical Requirements Significant amount of standing, walking, bending, squatting, reaching, lifting, pushing, and pulling for prolonged periods. Ability to lift/exert up to 50 lbs. Possible exposure to bodily fluids. Visual acuity and acute hearing required for accurate patient assessment and documentation. Sufficient manual dexterity to operate equipment and computer keyboard.
