Loudoun Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing home care team. The LPN will provide skilled nursing care to clients in their homes, coordinate care, and ensure services are delivered safely and in accordance with the individualized plan of care, physician orders, agency policies, and applicable Virginia regulations under the supervision of a Registered Nurse

General Accountabilities

• Provide nursing care as directed by the plan of care and supervising RN.
• Perform treatments, administer medications, and complete procedures as permitted by law and agency policy.
• Observe, record, and report client status, including changes that may require RN/physician intervention.
• Accurately document all services provided in the client record.
• Assist in implementing infection control practices and safety measures in client care.
• Support the RN in supervising home health aides by reporting aide performance and client concerns.
• Participate in client and family education under RN direction.
• Maintain confidentiality of all client information in compliance with HIPAA and Virginia law.
• Participate in quality improvement and staff training activities as assigned.

Job Qualifications

• Current Licensed Practical Nurse (LPN) license in Virginia/Compact
• Minimum one year of nursing experience preferred (home health or community health desirable).
• Ability to recognize and report significant changes in client condition.
• Knowledge of infection control, medication administration, and home health best practices.
• Strong interpersonal and documentation skills.
• Current CPR/BLS certification preferred or required within agency guidelines.
• Experienced with G-tube and Trach.
