Monitors utilization of resources of all patients reviewed. Maintains confidential communication with physicians and multi-disciplinary personnel. Responsible for coordinating the delivery of services for a select group of patients by providing assistance and support to appropriate members of the health care team. The case manager expedites value-added interventions to achieve desired outcomes, and assists in collaboration, research, development, implementation, revision and reporting of case management initiatives. Performs miscellaneous duties as assigned. Qualifications Minimum Education / Licensure / Certifications Graduation from Nursing or Medical School For Nursing School graduates, Registered Nurse license Minimum Experience Previous utilization, discharge planning and Case management including exposure to Interqual criteria.
