Clinical Case Manager - Heart and Vascular Services

Recruiter Contact: Taryn Blydenburgh at View email address on us.fitly.work SUMMARY OF POSITION: Responsible to coordinate interdisciplinary professional care of a select patient population, Vascular Surgery, as determined by individualized plans of care and clinical pathways. Functions include but not limited to RN triage, patient phone calls and follow up, care coordination, patient intake, insurance authorization, and medical record reviews. The expectation is that the RN will report to various clinics to assist the Attending with wound care, patient teaching, and coordination of follow up care. Attendance and participation in weekly education. This is very much an in-person clinical position which requires a fundamental knowledge of Vascular patients, with preference to Vascular Surgery and wound care. MINIMUM QUALIFICATION(S):
Two (2) years nursing experience
Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
AHA BLS prior to end of orientation period

PREFERRED QUALIFICATION(S):
Previous Vascular Surgery/wound care experience.
Outpatient experience preferred.
