Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients referred to the hospice program, identifying emotional, social, and financial needs to create individualized care plans.

• Engage in individual and group counseling sessions with patients and families, addressing issues related to grief, loss, and coping with terminal illness.

• Collaborate with healthcare professionals to develop and implement comprehensive discharge plans that incorporate community resources and referrals for ongoing support.

• Deliver crisis intervention and emotional support in high-stress situations, ensuring timely responses to patients in distress.

Qualifications\n
