RN Clinical Case Manager - Bilingual

RN Clinical Case Manager - Bilingual Job Overview Manage clinical services for home health patients, supervise home care personnel, and ensure compliance with state and federal regulations. Responsibilities Supervise all home care personnel and enforce compliance with regulations. Coordinate implementation of each patient's Plan of Care and maintain effective communication. Instruct, train, and re‑educate staff on clinical skills and procedures. Provide direct supervision of field LVNs/LPNs/RNs per state guidelines. Support, guide, and educate patients, families, and caregivers to maintain optimal physical and emotional well‑being. Communicate all occurrences and pertinent information to the Clinical Supervisor/Director of Clinical Services. Report any observed or suspected child/adult abuse, neglect, or exploitation to proper authorities. Collaborate with Clinical Supervisor and Director of Clinical Services to resolve client/clinical concerns promptly. Develop and implement treatment plans for each patient under physician direction. Complete clinical documentation (admissions, re‑certifications, resumptions) in a timely manner. Review clinical documentation to ensure completeness and compliance with payer source requirements. Interview and evaluate potential clinical field staff for competency. Participate in quality improvement, risk management, and incident reporting. Attend continuing education in‑services and conferences to maintain clinical competency. Represent the company positively in professional and community settings. Adhere to dress code, punctuality, and company policies. Qualifications Current state license as a Registered Nurse (RN). BSN is preferred. Minimum two years of nursing experience in home health or acute care (preferred). Strong leadership, supervisory, and interpersonal skills. Detail‑oriented, flexible, with good organizational and time‑management skills. Knowledge of cost management for clinical services. Valid CPR card and current health certificate, if applicable. State driver’s license and automobile insurance. Benefits Base salary of $75,000 – $80,000. Monthly cell phone and mileage reimbursements. Health, dental, and vision coverage.
