About the Role:

As an RN Case Manager , you will be at the forefront of in-home hospice care, ensuring patients receive the compassionate and comprehensive support they need. Your role will involve collaborating with healthcare professionals to design and implement care plans, maintaining meticulous records, and being an advocate for your patients and their families.

What We're Looking For:

- A valid RN license in Colorado .

- A minimum of 1 year of experience in supporting in-home hospice patients.

- Strong communication and interpersonal skills to effectively convey care plans and support patients and their families.

- The ability to work both independently and as part of a multidisciplinary team.

- Reliable transportation for field visits within the Greater Denver area.
