Full-Time

Pay: $44,000-$48,000

The Role

For people experiencing homelessness, getting to a doctor, filling a prescription, or keeping Medicaid active can feel impossible. As our Medical Case Manager, you'll make it possible. Reporting to the Director of SEEDS Case Management, you'll help guests access and stay connected to healthcare while providing the broader case management that moves them toward stability and housing.

What you'll do:

· Serve as the primary liaison with local pharmacies, calling in refills, picking up and delivering medications, and making sure pharmacy bills are paid

· Connect guests with primary care through partners such as Aunt Martha's

· Schedule medical, dental, and behavioral health appointments, and arrange transportation and follow-up to ensure continuity of care

· Check in regularly with guests to support medication adherence and refills

· Help guests enroll in Medicaid and reinstate benefits

· Advocate for guests with hospitals, clinics, and insurance providers

· Conduct intake assessments, and use the MAP assessment to identify appropriate housing placements

· Set goals with guests, provide case management and counseling, and track progress and case notes in WellSky

· Help guests obtain essential documents such as state IDs, Social Security cards, and birth certificates

· Partner with outside agencies to connect guests with employment, mental health, substance use treatment, legal, and housing services

· Attend program, all-staff, and supervision meetings, and support other Hesed House programs as needed

What You Bring

· Bachelor's degree in social work or a related human services field

· Strong verbal and written communication skills

· Solid computer skills and comfort with case management software

· Commitment to Harm Reduction and Housing First approaches

· Commitment to protecting guests' confidential health and personal information, with professionalism and healthy boundaries

· Ability to climb stairs regularly and lift up to 50 lbs

· Ability to pass DCFS and criminal background checks

· Experience with people experiencing homelessness or other vulnerable populations preferred, familiarity with Medicaid or healthcare navigation, basic first aid training, "street smarts," and Spanish/English bilingual skills a plus
