A growing healthcare technology company is seeking a Community Health Worker to support patient engagement, care coordination, and enrollment efforts focused on improving cardiovascular care.
Role Highlights
- Hybrid position: 2–3 days remote / 2–3 days in the community
- Travel to hospitals and healthcare facilities throughout Chicagoland
- Engage patients in person, by phone, text, and email
- Educate patients about available care programs and support enrollment
- Build relationships with patients, caregivers, hospital teams, and clinical staff
- Help coordinate discharge plans and successful transitions back home
- Maintain patient documentation and activity within the company's EHR
- Collaborate with an interdisciplinary healthcare team
- Meet monthly outreach, enrollment, and engagement goals
Ideal Background
- 3+ years of healthcare experience in a hospital, clinic, physician practice, or similar environment
- Background as a Community Health Worker, Medical Assistant, LPN, CNA, or similar role
- Cardiology experience preferred
- Strong patient communication and relationship-building skills
- Comfortable explaining healthcare programs and engaging patients
- Tech-savvy and able to learn new systems quickly
- Care coordination or interdisciplinary team experience preferred
- Telehealth experience is a plus
- Spanish-speaking is a plus
- Valid driver's license, reliable transportation, and ability to travel locally
- Available Monday–Friday, 8:00 AM–5:00 PM
