AmeriCorps Health Educator - Health Promotions (Hybrid)

Main Duties & Responsibilities Provide one‑on‑one health education and self‑management support to patients Participate in community events, training, and AmeriCorps service activities Support Lending Hands for Life HIV programming and re‑engagement efforts Conduct outreach and engagement for patients requiring additional support Document patient interactions and coordinate follow‑up care activities Facilitate health education groups and recruit participants for programs Applications must be submitted by September 13, 2026, to be considered for full‑time opportunities. Anticipated start of Monday, September 21, 2026. Qualifications High school diploma or equivalent. Strong verbal and written communication skills. Ability to work with diverse populations. Current CPR Certification. U.S. citizen, national, or lawful permanent resident. Ability to meet AmeriCorps background check requirements. Preferred Experience in community health, social services, or related fields. Intermediate Microsoft Office skills. Experience with electronic health record systems.
