Home Health Physical Therapist

This is a home health Physical Therapy position. The manager will consider applicants with no home health experience. Visits will involve an average of six patients per day. There is no call requirement for this position. Occasional weekend work may be needed. The unit utilizes Epic for documentation and communication. Primary responsibilities include providing initial assessments and vital sign monitoring. Treatments may involve fall risk reduction training, balance and strengthening exercises, gait and transfer training, and stair training. Collaboration with a multidisciplinary team is essential to achieve discharge goals. Equipment needs must be assessed during patient visits. Skills in various areas of physical therapy are required, including assessment and adaptive devices, as well as experience with chronic conditions. This position focuses on an adult and geriatric population with various acute, subacute, and chronic diagnoses.

Experience: 1 year required License: New Hampshire license required; will accept pending license if obtained by start date Certifications: BLS, Manual Therapy, Heart Saver CPR equivalent Must-Have: Experience with home health preferred, but first-time travelers will be considered
