Shift Worked: Varies Weekly Hours: 15 FLSA Status: Non Exempt Pay range: $18 - $20 The Community Habilitation Specialist works directly with individuals with intellectual and developmental disabilities, delivering Medicaid Waiver Services in community-based settings. This role focuses on supporting individuals in achieving their personal outcomes by building skills that promote independence, inclusion, and overall well-being. Position Duties • Responsibilities include, but are not limited to: • Provide direct support to individuals with intellectual and developmental disabilities to enhance daily living, vocational, and decision-making skills. • Deliver training aligned with each individual's Staff Action Plan and Life Plan. • Support skill development in areas such as self-care, independent living, community integration, and employment readiness. • Promote health and safety by delivering services in the least restrictive and most supportive manner. • Assist individuals in achieving goals outlined in their Person-Centered Plan (PCP). • Complete all required documentation accurately and in compliance with federal and state regulations. • Maintain consistent service delivery according to assigned schedules. • Collaborate with interdisciplinary team members to support participant progress and outcomes. • Respond effectively to crisis or emergency situations to ensure safety and well-being. • Monitor and report changes in individuals' physical or emotional condition. • Participate in the development, implementation, and evaluation of individualized service plans. • Assist in assessing individual strengths, needs, and skill development. • Contribute to program growth by identifying and engaging potential participants. • Maintain professional communication skills (verbal and written) and basic computer proficiency (e.g., Microsoft Word). • Travel as required throughout Putnam County and Southern Dutchess County. • Ensure full compliance with all applicable laws, regulations, and agency policies. • Complete required compliance training annually and cooperate with audits or investigations. • Strong interpersonal and communication skills • Ability to work independently and as part of a team • Commitment to person-centered care and inclusion • Organizational and documentation skills • Crisis management and problem-solving abilities • Adhere to the agency's Corporate Compliance Program, including the Code of Business Conduct, New York State and federal False Claims Acts, whistleblower protections, and mandatory reporting requirements • Perform additional duties as assigned. Requirements • High School Diploma or GED preferred. • Minimum of two (2) years' experience working with individuals with developmental disabilities preferred.
