We are seeking a dynamic, relationship-driven Hospice Liaison to help connect patients and families with the hospice services they need while building strong partnerships throughout the healthcare community.

What You'll Do
As a Hospice Liaison, you will serve as the primary representative of Medcare Hospice within your assigned territory. You will be responsible for driving referral growth, strengthening community partnerships, and ensuring patients have timely access to hospice care.
Key responsibilities include:
- Developing and maintaining relationships with hospitals, physicians, skilled nursing facilities, assisted living facilities, home health agencies, and other referral partners.
- Educating healthcare professionals on hospice services, eligibility guidelines, and benefits of hospice care.
- Conducting in-services, presentations, and community outreach activities.
- Collaborating with Admissions, Intake, and Clinical teams to facilitate seamless patient referrals and admissions.
- Ensuring referral documentation is complete and supports hospice eligibility requirements.
- Maintaining accurate CRM and activity records.
- Creating and executing territory growth strategies.
- Meeting or exceeding referral, admission, and business development goals.
- Serving as a trusted resource to referral partners and healthcare professionals.
- Representing Medcare Hospice with professionalism, compassion, and integrity.
What We're Looking For
Required Qualifications
- Minimum two years of professional sales experience.
- Strong relationship-building and networking abilities.
- Excellent communication, presentation, and public speaking skills.
- Ability to work independently and manage a territory effectively.
- Valid driver's license and reliable transportation.
- Willingness to travel throughout the assigned service area.
- Strong organizational and time management skills.
- Proficiency with Microsoft Outlook, Word, Excel, PowerPoint, and CRM systems.
Preferred Qualifications
- Healthcare, hospice, home health, or medical sales experience.
- Bachelor's degree in a related field.
- Bilingual English/Spanish.
- Knowledge of hospice eligibility guidelines and referral processes.
