Location 34632 - East Valley Ambulatory About the Organization Lifeline Vascular Care works with premier physician practices to develop and manage outpatient centers that provide focused and coordinated vascular access procedures to patients receiving hemodialysis treatment for end-stage renal disease (ESRD). We currently manage more than 40 physician-owned vascular access centers where we provide physician practices with financial and outcomes reporting, staffing and training, facility operations, regulatory compliance, and insurance billing services. The Lifeline network of managed vascular access centers is accredited by the Joint Commission. Through our relationships with nephrologists, vascular surgeons, interventional radiologists and interventional cardiologists, we create and fortify environments in which these crucial services are delivered both seamlessly and efficiently. GENERAL PURPOSE OF THE JOB
The Care Coordinator is responsible for managing and streamlining the continuum of vascular access care for patients requiring dialysis. This role serves as a key liaison between patients, nephrologists, surgeons, dialysis facilities, and other care team members. The Care Coordinator ensures timely scheduling, follow-up, and communication for all stages of vascular access, from initial catheter placement through long-term fistula or graft maintenance while promoting patient education and positive health outcomes. The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive. Patient Navigation & Scheduling Serve as the central point of contact for patients and care teams throughout the AV access process. Coordinate referrals and schedule appointments for vein mapping after initial catheter placement. Arrange pre-surgical clearance and scheduling for AV fistula or graft creation. Schedule surgical procedures and confirm all pre-operative requirements are met. Post-Surgical & Ongoing Care Coordination Monitor fistula maturation progress through follow-up communication and imaging when needed. Coordinate evaluation and clearance for cannulation readiness. Facilitate catheter removal once the fistula/graft is ready for use. Maintain ongoing surveillance of long-term fistula or graft health, liaising with dialysis teams, nephrologists, surgeons, and endovascular specialists (EVASC). Documentation & Communication Maintain detailed and accurate patient records in the EHR. Communicate proactively with patients, families, and providers regarding timelines, procedures, and expectations. Track milestones in the AV access workflow and ensure timely interventions at each stage. Quality & Compliance Ensure adherence to clinical protocols, regulatory standards, and HIPAA requirements. Identify workflow improvements and participate in quality initiatives related to vascular access management.MINIMUM QUALIFICATIONS
Education, licenses, certifications, and experience required to fulfill the essential duties, including computer skills as required. Nephrology experience preferred. Experience in teaching the adult learner strongly preferred with demonstrated ability to communicate vascular access/medical information in lay terms as necessary. Scheduling flexibility Current driver's license in state of residence and insurable driving record required.
