About the Role
A leading regional healthcare provider known for patient-centric care is seeking a Home Health Registered Nurse to join its field clinical team. The organization delivers skilled nursing, rehabilitation, and hospice care to patients in their homes across the region, and it is known for manageable caseloads and strong clinical support. Full-time and per diem schedules are available, with a predictable weekday routine and a territory close to home. This role suits an RN who works independently, enjoys teaching patients and families, and wants meaningful one-on-one care outside the hospital.

Clinicians here are supported by a clinical manager, intake and scheduling teams, and a collaborative interdisciplinary group that meets regularly to review patient progress. New nurses complete a structured orientation that includes field ride-alongs and documentation training, and the organization is committed to realistic productivity expectations so nurses can deliver thorough, high-quality visits.

Core Responsibilities

• Conduct start of care, recertification, resumption, and discharge visits in patient homes
• Complete comprehensive OASIS assessments accurately and on time
• Develop and update individualized plans of care in partnership with physicians and the care team
• Provide skilled nursing interventions including wound care, IV therapy, medication management, and disease management
• Teach patients and caregivers about medications, symptoms, safety, and self-management
• Coordinate with therapy, aides, social work, and physicians to keep care on track
• Document visits in the electronic medical record within required timeframes
• Identify changes in condition and escalate promptly
• Support quality, compliance, and patient satisfaction goals
• Participate in case conferences and ongoing clinical education
• Supervise home health aides and verify that aide care plans are followed
• Participate in quality improvement activities and help patients avoid unnecessary hospital readmissions

Required Qualifications

• Active, unrestricted RN license in the state of practice or an eligible compact license
• Current BLS or CPR certification
• One year or more of RN experience, with home health experience strongly preferred
• OASIS experience preferred; training is available for strong clinicians new to home health
• Valid driver license, reliable transportation, and current auto insurance
• Comfort with point-of-care documentation on a tablet or laptop
• Strong assessment, time management, and critical thinking skills
• Commitment to patient-centered care, privacy, and professional boundaries

Benefits & Pay
Pay: $42.00 - $55.00 per hour, based on experience and schedule.
