MAJOR RESPONSIBILITIES

Provide program support to the Health Home Program by working with the Care Coordinators and Director of Health Homes.

DETAILED RESPONSIBILITIES

• Maintain records within Captain (close members, assign goals, etc.)
• Create member files
• Take inventory and order office/kitchen supplies as needed
• Distribute ordered supplies and restock supplies as required in designated areas
• Provide office orientation for new staff (introduce to other teams, show/review emergency exits, show/review safety protocol)
• Send a program welcome letter to new members and closing documents to members who have disenrolled
• Organize and deliver inter-office mail
• Send member satisfaction surveys
• Answering Phones, greeting visitors, Scanning, faxing, and filing as required
• Ability to lift, stand, bend, and sit to complete needed tasks

QUALIFICATIONS:

• MUST HAVE Computer knowledge, including but not limited to Windows, Microsoft Office
• Ability to work as part of a team with co-workers and with other colleagues as necessary
• Cultural sensitivity and the ability to relate/work with diverse groups, community agencies, schools, and the general public
• Strong organizational skills with a focus on detail
• Ability to Multitask in face pace environment
• Ability to follow directions and communicate well (orally and in writing)

EDUCATION/TRAINING/EXPERIENCE

• High School Diploma/GED Plus two years of experience as a secretarial or program assistant.
• Bi-lingual (Spanish) preferred.

• A Valid Driver's License and Insured Vehicle.
• PPD (TB test required with proof of results; no more than 1 year old)
