Home Health LPN / Licensed Practical Nurse

Healthcare Support is actively seeking a Home Health Licensed Practical Nurse to fill an opening for a top healthcare provider located in Scranton, PA.

Daily Responsibilities for the Home Health LPN / Licensed Practical Nurse:

Home healthcare nurses provide follow-up care to patients after they have been discharged from a hospital or rehabilitation, skilled nursing, or long-term care facility
Provide nursing visits in accordance with the written plan of treatment signed by the physician and all subsequent orders that authorize and direct changes
Provide (on a daily basis) paperwork as required by agency to maintain accurate and complete medical records
Conduct and pursues the business of agency in accordance with sound nursing and medical practices and represents agency in a manner calculated to instill confidence in patients, physicians and the general public

Shift: 32 hours - 4 days/week - set day off will either be Tuesday or Thursday (May be flexible if needed)

Required Qualifications for the Home Health LPN / Licensed Practical Nurse:

Graduate of accredited practical/vocational nursing program
Currently Licensed Practical/Vocational Nurse in Pennsylvania
Must possess and maintain valid CPR certification while employed in a clinical role
At least 1-year acute care experience as an LVN (Home Health, Med-Surg, ED, SNF etc.)
Experience with IVs, Wound Care, and Disease Management (diabetes, CHF, liver disease, COPD)
Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order

Pay Details:

Position is Pay Per Visit however annual salary ranges between $55-62K for 32 hours per week
$10k sign-on bonus!
