Clinical Care Manager

VitalCaring is seeking a Clinical Care Manager to lead and coordinate home health and hospice services in patients’ homes. In this RN-led role, you’ll oversee care plans, supervise field clinicians, and ensure high-quality, compassionate, and compliant care. You’ll conduct assessments, monitor outcomes, educate patients and families, and collaborate with an interdisciplinary team.
