Hybrid Facility-Based Care Manager II: Transitional Care

Alliance Health

Alliance Health is seeking a Care Manager II-Facility Based to provide Transitional Care Management and Physical Health Consultation for members in the North Carolina area. This full-time hybrid position involves supporting members during their discharge planning and coordinating care for various health needs. The ideal candidate will hold a Master's degree in Human Services or be a Nursing graduate with substantial care management experience.
