Conflict-Free Case Manager - Elder & Disabled Services

Tri-county Community Action Agency

Tri-County Community Action Agency is seeking a full-time Case Manager located in Johnston, RI. This role focuses on case management activities for Elder Adults Disabled populations on Medicaid, ensuring comprehensive care and coordination of services. The successful candidate will develop person-centered plans, conduct regular participant monitoring, and maintain oversight of service provision to enhance participant wellbeing.
