Residential Case Manager Grandview Apartments $2,000 retention bonus

Residential Case Manager Grandview Apartments $2,000 retention bonus This position is eligible for a prorated $2,000 retention bonus. Job Summary: The Providence Center Residential Case Manager Grandview Apartments will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible. Schedule: Sunday 3:00pm-11:00pm, Monday 3:00pm-11:00pm, Tuesday 3:00pm-11:00pm, Saturday 7:00am-3:00pm. 2nd shift, 3rd shift, weekends, and holidays eligible for shift differential compensation. Duties and Responsibilities: Initiate treatment and advocate for consumer within network of care. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy. Provide case management and support to consumers. Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment. Teach consumers adult daily living skills, assisting them in all aspects of their life in the group home and supervised apartments. Transport consumers in own vehicle or Center van to planned activities and appointments. Maintain cooperative relationships with Center staff, consumers, community agencies and the public. Work flexible hours, including evenings, weekends, holidays and during inclement weather when unforeseen circumstances arise to ensure 24-hour coverage. Other duties may be assigned. Requirements: High School Diploma/GED required and/or at least 5 years of experience in a behavioral health setting. Experience in residential facilities is preferred. Must have valid driver's license, insured auto, and personal vehicle. Tier II drivers must maintain current inspection and registration, insurance must meet minimum limits as required by the State of RI (currently $25K/$50K/$25K) per CNE Motor Vehicle Operation policy CNE-RM-003.
