Responsibilities
- Provide direct physical therapy services to home health clients, including initial evaluations, ongoing assessments, and development of a comprehensive plan of care in collaboration with the patient, family, and care team.
- Implement and document rehabilitation interventions, including therapeutic exercises, gait and balance training, ADL optimization, and utilization of modalities as appropriate; apply splints, orthoses, prostheses, and assistive devices when indicated.
- Ensure accurate, timely documentation of progress, alignment with the plan of care, and coordination of care with nursing, social work, and other therapy disciplines.
- Educate patients and caregivers on home exercise programs, safety, fall prevention, wheelchair and ADL equipment use, and strategies to maximize independence in the home.
- Maintain safe operation and care of rehabilitation equipment and supplies; participate in discharge planning and transitions of care as patient needs evolve.
- Communicate findings and recommendations effectively with multidisciplinary team members to support optimal patient outcomes and quality of life in the home setting.

Requirements
- Active Massachusetts Physical Therapist license
- BLS certification (American Heart Association)
- Home Health Certification required; proficiency with OASIS data collection and documentation required
- Minimum 2+ years of combined acute hospital care and home health/home care experience
- Valid driver’s license and access to reliable transportation; ability to travel between patient locations
- Ability to work independently with minimal supervision and to collaborate effectively with an interdisciplinary team
