Base pay range $45.00/hr - $50.00/hr Duties Conduct biopsychosocial assessments to collect and analyze patient information. Develop and implement plans of care to meet patient and caregiver needs. Collaborate with a multi-disciplinary team to ensure continuity of care. Educate patients and caregivers to facilitate discharge and promote understanding of care plans. Provide patient and family education, support, and advocacy. Facilitate discharge and contingency planning. Document patient care, including delivery, plans, goals, and patient responses, according to plan of care. Skills Biopsychosocial assessment Provide patient and family education, support, and advocacy Work with a multi-disciplinary team to ensure continuity of care Knowledge of psychiatric hospital settings Understanding of short-term therapy utilization Knowledge of protective services, such as adult and child protective services Management/lead experience Familiarity with community programs, including Medicaid, Medicare, and social services Experience Minimum of 2 years of experience in a relevant field. Experience with the EPIC charting system is preferred. Community hospital experience is preferred.
