Location: Smyrna, DE 19977

Schedule: Mon., Tues., Wed.: 8:00am - 4:00pm; Thurs.: 8:00am - 3:45pm

Role Summary

The Bilingual Case Manager is responsible for providing comprehensive case management services to individuals and families and ensuring timely access to essential supports. You will maintain accurate documentation, meets regulatory requirements, and collaborates with internal teams and community partners to promote client stability and well-being. Because a significant portion of residents primarily speak Spanish, the Case Manager provides bilingual communication to ensure equitable access to services, reduce language barriers, and support clear, compassionate interactions between residents and staff.

Key Responsibilities

  • Conducting assessments to determine clients' needs for a variety of health and human services.
  • Develop individualized, comprehensive, and cost-effective service plans.
  • Coordinate resources using in-house sources, referrals, and contracted providers.
  • Maintains accurate case records, summaries, statistical and narrative reports.
  • Build supportive relationships with clients, families, and community partners.
  • Communicate effectively with individuals from diverse backgrounds and abilities.

Required Qualifications

To be considered, candidates must meet all of the requirements below:

  1. Bachelors degree or higher in Behavioral or Social Science or related field.
  2. Read, write and speak Spanish fluently.
  3. At least one (1) year experience of working as a supervised social worker in a health care setting, such as working directly with individuals in a health care facility.
